A ministry candidate is asking that you complete the following reference form. This candidate has completed the basic requirements for ordination with the Florida Disciple Regional Church, with the Commission on Ministry. To complete our recommendation for ordination, we ask for a letter of reference from the pastor of a church who is familiar with the candidate’s journey toward ordination and who will sponsor this candidate for ordination. Would you please fill out the following form and press the submit button at the bottom. This completed form will be sent to the COM Chairperson, Donna Oberkreser and you will receive a confirmation email that includes this form. Thank you!
