A ministry candidate is asking that you complete the following reference form as they are applying for candidacy with the Commission on Ministry of the Christian Church, Disciples of Christ, through the Florida Disciples Regional Church. “Candidacy” is not approval for ordination but is an entrance requirement in a process of oversight, care, and certification leading to a consideration for approval for ordination at the end of either the Seminary or Apprentice Track. They have given you as a reference. Please fill out and click on the Submit button. Once submitted the form will be sent to Donna Oberkreser, the COM Chair. Thank you.
